Community Wellness Navigator Program Overview - Weightloss

$250.00 every month for 1 year

Welcome to our Community Wellness Navigator Program!
This program is designed to support groups within the community in achieving holistic health, wellness, and sustainable weight loss together. We understand the importance of nurturing a supportive dynamic within community groups for overall well-being. Through personalized guidance, resources, and accountability, our program empowers community members to thrive physically, mentally, and emotionally.

Program Components:

  • Private Consultation
    We conduct comprehensive consultations with community groups to understand collective health goals, challenges, routines, and dynamics—including weight management goals.

  • Personalized Community Wellness Plan
    Based on consultations, we develop customized wellness plans incorporating nutrition, mindful movement, weight loss strategies, communication tools, and more.

  • Weekly Group Sessions
    Community groups participate in interactive Zoom sessions facilitated by our Wellness Navigators. These sessions review progress (including weight loss goals), discuss challenges and successes, explore strategies for improvement, and educate for prevention.
    Thursdays @ 2:00pm PST.

  • Individual Support
    Each member receives individualized support tailored to their unique wellness and weight loss goals.
